Statewide Office of Advance Care Planning

The Statewide Office of Advance Care Planning offers free information and support for patients, families, carers, and health professionals in Queensland.

They can help you:

  • understand advance care planning, forms and processes
  • find resources to help you talk to your family, attorneys and health professionals about your wishes
  • connect with support services in your area
  • check your documents are complete and properly signed or witnessed
  • upload your documents to the Queensland Health electronic hospital record (The Viewer).
